For everyone's safety, we abide by all health guidelines administered by the county health department, CDC, and the individual facility.
Need to Know
All signers must have a valid ID with their picture, description, signature, expiration/issue date, and serial number. If they don't (which happens with many seniors), arrangements must be made to have two credible witnesses present that will swear under oath to the identity of the signer (they must have their own valid ID). Neither witness can be named on the document to be signed.

Finally, the signer must be cognizant and aware of what they are signing. If they are not, the document can not be notarized.
